ABSTRACT

Objective: To analyze the short-term clinical effects of robot-assisted and laparoscopic repair of the hiatal hernia. Methods: The clinical data of 56 patients underwent minimally invasive hiatal hernia repair from January 2021 to January 2022 in the Department of Minimally Invasive Surgery, Hernias and Abdominal Wall Surgery, People's Hospital of Xinjiang Uygur Autonomous Region were retrospectively analyzed. There were 32 males and 24 females, aging (59.7±10.7) years (range: 28 to 75 years). All patients were divided into laparoscopy group (n=27) and robot group (n=29) according to surgical procedures. Perioperative conditions, hospital stay, and improvement in symptoms before and after surgery were compared between the two groups by the t test, Wilcoxon rank-sum test and χ2 test. Results: All surgical procedures were successfully completed, without conversion to laparotomy or change in operation mode. There were no serious complications related to the operation. The intraoperative blood loss of the robot group was less than that of the laparoscopic group (M (IQR)): (20 (110) ml vs. 40 (80) ml, Z=-4.098, P<0.01). The operation time ((111.7±33.6) minutes vs. (120.4±35.0) minutes, t=-0.943, P=0.350) and hospitalization time ((3.9±1.4) days vs. (4.7±1.9) days, t=-1.980, P=0.053) of the robot group and the laparoscopic group were similar. Follow-up for 12 months after the operation showed no postoperative complications and recurrence. The score of the health-related quality of life questionnaire for gastroesophageal reflux disease in the robot group decreased from 10.8±2.8 before the operation to 6.5±0.6 after the operation, and that in the laparoscopic group decreased from 10.6±2.1 before the operation to 6.3±0.6 after the operation. There was no difference in the influence of different surgical methods on the change in score (t=0.030,P=0.976). Conclusion: Compared with laparoscopic repair of the hiatal hernia, robot-assisted hiatal hernia repair has the advantages of less bleeding, rapid postoperative recovery and good short-term effect.

Resumen La fundoplicatura de Nissen laparoscópica (FNL) es el gold standard del tratamiento quirúrgico de pacientes con reflujo gastroesofágico patológico (RGE), con perfil de seguridad y resultados a largo plazo satisfactorios en un 80%. En la última década, se ha propuesto el uso de un dispositivo de potenciación del esfínter esofágico inferior (MSA) comercializado como LINX® (de Ethicon, Johnson & Johnson), como alternativa a la cirugía antirreflujo. El dispositivo consiste en una cadena expansible de cuentas de titanio con un núcleo magnético diseñadas para aumentar el tono del esfínter esofágico inferior (EEI) y evitar su relajación inadecuada. Los resultados preliminares han sido alentadores, sin embargo, no está exento de complicaciones. Las indicaciones más aceptadas son: RGE con motilidad esofágica normal y sin esofagitis erosiva, hernia hiatal < 3 cm, IMC <35 kg/m2 y pacientes sin cirugías gastroesofágicas previas. Se instala por vía laparoscópica. Los efectos adversos más importantes son: disfagia con necesidad de dilataciones endoscópicas, y, retiro del dispositivo, entre 1% y 7% a los 2,5 años. La mayoría de los estudios tienen limitaciones metodológicas, conflictos de interés, falta de resultados objetivos y seguimientos a largo plazo, impidiendo llegar a conclusiones extrapolables respecto a la eficacia del MSA.


Laparoscopic Nissen Fundoplication (LNF) is the gold standard of surgical treatment for patients with Gastroesophageal Reflux Disease (GER), with safety profile and long term results satisfactory in 80%. In the last decade, the use of a Lower Esophageal Sphincter Enhancement Device (LESD), marketed as LINX® (from Ethicon, Johnson & Johnson), has been proposed as an alternative to Anti-Reflux Surgery. The device consists of an expandable chain of titanium beads with a magnetic core designed to increase the tone of the lower esophageal sphincter (LES) and prevent its improper relaxation. Preliminary results have been encouraging; however, it is not without complications. The most accepted indications are: GER with normal esophageal motility and without erosive esophagitis, hiatal hernia < 3 cm, BMI <35 kg/m2, and patients without previous gastroesophageal surgeries. It is installed by laparoscopy. The most important adverse effects are: dysphagia with the need for endoscopic dilations, and, removal of the device, between 1 and 7% at 2.5 years. Most of the studies have methodological limitations, conflicts of interest, lack of objective results and long-term follow-up, preventing the reaching of extrapolable conclusions regarding the efficacy of MSA.

ABSTRACT Background: Gastroesophageal reflux disease is defined by the abnormal presence of gastric content in the esophagus, with 10% incidence in the Western population, being fundoplication one treatment option. Aim: To evaluate the early (six months) and late (15 years) effectiveness of laparoscopic fundoplication, the long term postoperative weight changes, as well as the impact of weight gain in symptoms control. Methods: Prospective study of 40 subjects who underwent laparoscopic Nissen's fundoplication. Preoperatively and early postoperatively, clinical, endoscopic, radiologic, manometric and pHmetric evaluations were carried out. After 15 years, clinical and endoscopic assessments were carried out and the results compared with the early ones. The presence or absence of obesity was stratified in both early and late phases, and its influence in the long-term results of fundoplication was studied, measuring quality of life according to the Visick criteria. Results: The mean preoperative ages, weight, and body mass index were respectively, 51 years, 69.67 kg and 25.68 kg/m2. The intraoperative and postoperative complications rates were 12.5% and 15%, without mortality. In the early postoperative period the symptoms were well controlled, hernias and esophagitis disappeared, the lower esophageal sphincter had functional improvement, and pHmetry parameters normalized. In the late follow-up 29 subjects were assessed. During this period there was adequate clinical control of reflux regardless of weight gain. In both time periods Visick criteria improved. Conclusion: Fundoplication was safe and effective in early and late periods. There was late weight gain, which did not influence effective symptoms control.


RESUMO Racional: A doença do refluxo gastroesofágico é definida pela presença anormal do conteúdo gástrico no esôfago com incidência de 10% na população ocidental, sendo a fundoplicatura uma das opções de tratamento. Objetivo: Avaliar a efetividade precoce (seis meses) e tardia (15 anos) da fundoplicatura laparoscópica, bem como a evolução ponderal pós-operatória em longo prazo, e o impacto do ganho de peso no controle tardio dos sintomas. Métodos: Estudo prospectivo com 40 indivíduos submetidos à fundoplicatura laparoscópica pela técnica de Nissen. No pré e pós-operatório precoce, foram realizadas avaliações clínica, endoscópica, radiológica, manométrica e pHmétrica. Após 15 anos, realizaram-se avaliações clínica e endoscópica, comparando-se os resultados com os da fase precoce. Estratificou-se a presença ou ausência de obesidade nestas fases e estudou-se sua influência nos resultados em longo prazo, mensurando-se a qualidade de vida pelos critérios de Visick. Resultados: As médias de idade, peso e do índice de massa corporal pré-operatórias foram 51 anos, 69.67 kg e 25,68 kg/m2. O índice de complicações intra e pós-operatórias foram 12,5% e 15%, sem mortalidade. No pós-operatório precoce houve controle dos sintomas, remissão das hérnias e esofagites, melhora funcional do esfíncter esofágico inferior e normalização dos parâmetros pHmétricos. No seguimento tardio, 29 indivíduos foram acompanhados. Nesta fase, houve adequado controle clínico do refluxo, independente do ganho de peso. Em ambas as fases houve melhora nos critérios de Visick. Conclusão: A fundoplicatura foi segura e efetiva, precoce e tardiamente. Houve ganho de peso tardio, o que não influenciou no controle efetivo dos sintomas.

ABSTRACT Objective: to evaluate esophageal dysmotility (ED) and the extent of Barrett's esophagus (BE) before and after laparoscopic Nissen fundoplication (LNF) in patients previously diagnosed with BE and ED. Methods: twenty-two patients with BE diagnosed by upper gastrointestinal (GI) endoscopy with biopsies and ED diagnosed by conventional esophageal manometry (CEM) were submitted to a LNF, and followed up with clinical evaluations, upper GI endoscopy with biopsies and CEM, for a minimum of 12 months after the surgical procedure. Results : sixteen patients were male (72.7%) and six were females (27.3%). The mean age was 55.14 (± 15.52) years old. and the mean postoperative follow-up was 26.2 months. The upper GI endoscopy showed that the mean length of BE was 4.09 cm preoperatively and 3.91cm postoperatively (p=0.042). The evaluation of esophageal dysmotility through conventional manometry showed that: the preoperative median of the lower esophageal sphincter resting pressure (LESRP) was 9.15 mmHg and 13.2 mmHg postoperatively (p=0.006). The preoperative median of the esophageal contraction amplitude was 47.85 mmHg, and 57.50 mmHg postoperatively (p=0.408). Preoperative evaluation of esophageal peristalsis showed that 13.6% of the sample presented diffuse esophageal spasm and 9.1% ineffective esophageal motility. In the postoperative, 4.5% of patients had diffuse esophageal spasm, 13.6% of aperistalsis and 22.7% of ineffective motor activity (p=0.133). Conclusion: LNF decreased the BE extension, increased the LES resting pressure, and increased the amplitude of the distal esophageal contraction; however, it was unable to improve ED.


RESUMO Objetivo: avaliar a dismotilidade esofágica (DE) e a extensão do esôfago de Barrett (EB) antes e depois da fundoplicatura laparoscópica a Nissen (FLN) em pacientes previamente diagnosticados com EB e DE. Método: vinte e dois pacientes com EB diagnosticada por endoscopia digestiva alta (EDA) com biópsias e DE diagnosticada por manometria esofágica convencional (MEC) foram submetidos a FLN, e acompanhados por avaliações clínicas, endoscopia digestiva alta com biópsias e MEC, por no mínimo 12 meses após o procedimento cirúrgico. Resultados: dezesseis pacientes eram do sexo masculino (72,7%) e seis do feminino (27,3%). A média de idade foi de 55,14 (± 15,52) anos e o seguimento pós-operatório médio foi de 26,2 meses. A endoscopia digestiva alta mostrou que o comprimento médio do EB foi de 4,09 cm no pré-operatório e 3,91 cm no pós-operatório (p = 0,042). A avaliação da dismotilidade esofágica por meio da manometria convencional mostrou que a mediana pré-operatória da pressão de repouso do esfíncter esofágico inferior (PREEI) foi de 9,15 mmHg, e de 13,2 mmHg no pós-operatório (p = 0,006). A mediana pré-operatória da amplitude de contração esofágica foi de 47,85 mmHg, e de 57,50 mmHg no pós-operatório (p = 0,408). A avaliação pré-operatória do peristaltismo esofágico mostrou que 13,6% da amostra apresentava espasmo esofágico difuso e 9,1%, motilidade esofágica ineficaz. No pós-operatório, 4,5% dos pacientes apresentaram espasmo esofágico difuso, 13,6% de aperistalse e 22,7% de atividade motora ineficaz (p = 0,133). Conclusões: a FLN diminuiu a extensão do EB, aumentou a pressão de repouso do EEI e aumentou a amplitude da contração esofágica distal; no entanto, não foi capaz de melhorar a DE.

RESUMEN Introducción: En la actualidad, la fundoplicatura laparoscópica se considera el tratamiento de elección para la enfermedad por reflujo gastroesofágico con resultados excelentes en más del 90 por ciento de los pacientes. Sin embargo, a pesar de estos resultados, el 30 % de los pacientes presentan persistencia de los síntomas y del 3 por ciento al 10 por ciento requieren reintervención por fallo de la cirugía antirreflujo. Objetivo: Describir las causas del fracaso de la cirugía antirreflujo y las técnicas realizadas en la reintervención. Método: Se realizó un estudio descriptivo, retrospectivo y longitudinal, de una serie de pacientes a los cuales se les realizo cirugía antirreflujo en el Centro Nacional de Cirugía de Mínimo Acceso desde enero de 1994 hasta diciembre de 2016. Las variables analizadas fueron: reintervenciones y sus causas, tratamiento quirúrgico, morbilidad y el índice de conversión. Resultados: De un total de 1 550 pacientes operados, 37 (2,3 por ciento) fueron reintervenidos. Las causas más frecuentes de reintervención fueron la recidiva de los síntomas y la presencia de disfagia. La herniación de la fundoplicatura fue el hallazgo transoperatorio más frecuente. El índice de conversión fue bajo y no hubo fallecidos en las reintervenciones. La morbilidad triplicó la del total de la serie, así como la estadía hospitalaria. Conclusiones: Las reintervenciones por fallo de la cirugía antirreflujo resultan de gran complejidad pues aumentan considerablemente la morbilidad y la estadía hospitalaria. Debe realizarse en centros que acumulen una alta experiencia en estas técnicas(AU)


ABSTRACT Introduction: Nowadays, laparoscopic fundoplication is considered the treatment of choice for gastroesophageal reflux disease, showing excellent results in more than 90 percent of patients. However, despite these results, 30 percent of patients present with persistent symptoms, while 3 percent to 10 percent require reintervention for failed antireflux surgery. Objective: To describe the causes of failed antireflux surgery and the techniques performed in the reintervention. Method: A descriptive, retrospective and longitudinal study was conducted with a series of patients who underwent antireflux surgery at the National Center for Minimally Access Surgery, from January 1994 to December 2016. The variables analyzed were reinterventions and their causes, surgical treatment, morbidity and the conversion rate. Results: From among 1550 patients operated on, 37 (2.3 percent) were reintervened. The most frequent causes of reintervention were the recurrence of symptoms and the occurrence of dysphagia. The fundoplication herniation was the most frequent transoperative finding. The conversion rate was low and there were no deaths in the reinterventions. The morbidity tripled that of the total of the series, as well as hospital stay. Conclusions: Reinterventions for failed antireflux surgery are very complex, since they increase morbidity and hospital stay considerably. It must be performed in centers that accumulate a high experience in these techniques(AU)

ABSTRACT

ABSTRACT Epiphrenic diverticulum is a rare disease associated with esophageal motor disorders that is usually asymptomatic and has a well-established surgical indication. We report a case of giant epiphrenic diverticulum in a 59-year-old symptomatic woman who was diagnosed after underwent complementary exams. Because of her symptoms, the surgical treatment was chosen, and esophageal diverticulectomy was performed along with laparoscopic cardiomyotomy and anterior partial fundoplication.


RESUMO O divertículo epifrênico é uma patologia rara associada a distúrbios motores esofágicos e, frequentemente, assintomática, tendo indicações cirúrgicas bem estabelecidas. Relatamos um caso de divertículo epifrênico gigante em paciente de 59 anos, sexo feminino, sintomática, diagnosticada por exames complementares. Devido à sintomatologia, optou-se por tratamento cirúrgico, sendo realizada a diverticulectomia esofágica com cardiomiotomia e fundoplicatura parcial anterior laparoscópica.

Resumen Introducción: En pacientes con esófago de Barrett largo hemos sugerido efectuar fundoplicatura con antrectomía, vagotomía y derivación duodenal en Y de Roux que podría asociarse con complicaciones y efectos colaterales. Objetivo: El objetivo de este estudio es comparar la cirugía por vía abierta vs laparoscópica en cuanto a complicaciones postoperatorias precoces y alejadas, mortalidad y resultados alejados. Material y método: Se comparan 2 cohortes de pacientes, 73 pacientes con cirugía abierta y 53 pacientes operados con la misma técnica por vía laparoscópica por el mismo equipo. Solo se incluyeron los pacientes con Barret largo. Se controlan clínicamente en el postoperatorio inmediato y alejado, con endoscopia e histología anual, y se evalúan los resultados en cuanto a complicaciones precoces, alejadas y se analiza la calidad de vida y la satisfacción del paciente. Para el análisis se utilizó «t¼ de Student considerando un valor de p < 0,05 como significativo. Resultados: En cuanto a complicaciones precoces en ambos grupos no hubo diferencias significativas. No hubo mortalidad postoperatoria. En las complicaciones tardías las complicaciones totales no son significativamente diferentes entre ambos grupos (solo cambian sus causas y características) ni en cuanto a la clasificación de Visick y el puntaje de calidad de vida. Conclusión: La fundoplicatura con procedimiento de supresión ácida y derivación biliar por vía laparoscópica presenta similares resultados a corto y largo plazo que la cirugía abierta, pero con los beneficios de una cirugía mínimamente invasiva.


Abstract Introduction: In patients with long Barrett esophagus we have suggested to perform fundoplication with antrectomy, vagotomy and Roux-en-Y duodenal diversion however it could be associated with complications and side effects. Objective: The objective of this study is to compare open versus laparoscopic surgery for early and early postoperative complications, mortality and distant outcomes. Material and method: We compare 2 cohorts of patients, 73 patients with open surgery and 53 patients, who underwent laparoscopic surgery using the same technique. Only patients with Long Barrett were included. They are clinically monitored in the early and late postoperative period, with endoscopy and histology at long term follow-up (3-5 years). The results were evaluated in terms of early and late complications, the quality of life and patient satisfaction were analyzed. For the analysis we used t-student considering a P < .05 as significant. Results: As for early complications, there were no significant differences in both groups. There was no postoperative mortality. In the late complications, the total complications are not significantly different between the two groups (only their causes and characteristics changed) neither in terms of Visick's classification and the quality of life score Conclusion: The fundoplication, with laparoscopic acid suppression and duodenal diversion, presents similar short-term and long-term results than open surgery, with the benefits of a mini-invasive procedure.

ABSTRACT Background: All available treatments for achalasia are palliative and aimed to eliminate the flow resistance caused by a hypertensive lower esophageal sphincter. Aim: To analyze the positive and negative prognostic factors in the improvement of dysphagia and to evaluate quality of life in patients undergoing surgery to treat esophageal achalasia by comparing findings before, immediately after, and in long follow-up. Methods: A total of 84 patients who underwent surgery for achalasia between 2001 and 2014 were retrospectively studied. The evaluation protocol with dysphagia scores compared preoperative, immediate (up to three months) postoperative and late (over one year) postoperative scores to estimate quality of life. Results: The surgical procedure was Heller-Dor in 100% of cases, with 84 cases performed laparoscopically. The percent reduction in pre- and immediate postoperative lower esophageal sphincter pressurewas 60.35% in the success group and 32.49% in the failure group. Regarding the late postoperative period, the mean percent decrease was 60.15% in the success group and 31.4% in the failure group. The mean overall drop in dysphagia score between the pre- and immediate postoperative periods was 7.33 points, which represents a decrease of 81.17%. Conclusions: Reduction greater than 60% percent in lower esophageal sphincter pressurebetween the pre- and postoperative periods suggests that this metric is a predictor of good prognosis for surgical response. Surgical treatment was able to have a good affect in quality of life and drastically changed dysphagia over time.


RESUMO Racional: Todos os tratamentos disponíveis para a acalásia são paliativos e visam eliminar a resistência ao fluxo causada por esfíncter esofágico inferior hipertenso. Objetivos: Avaliar os fatores prognósticos positivos e negativos na melhora da disfagia e avaliar a qualidade de vida em pacientes submetidos à cirurgia para tratar a acalásia esofágica, comparando os achados antes, imediatamente após e tardiamente à operação. Métodos : Foram estudados retrospectivamente 84 pacientes submetidos à operação de acalásia entre 2001 e 2014. O protocolo de avaliação com escores de disfagia comparou os escores pré-operatórios, do pós-operatório imediato (até três meses) e pontuação pós-operatória tardia (acima de um ano) para estimar a qualidade de vida. Resultados: O procedimento cirúrgico foi Heller-Dor em 100% dos casos, com 84 casos laparoscópicos. A redução percentual na pressão do esfíncter esofágico inferior pré e pós-operatório imediato foi de 60,35% no grupo de sucesso e de 32,49% no grupo de falha. Em relação ao período pós-operatório tardio, a redução percentual média foi de 60,15% no grupo de sucesso e 31,4% no grupo de falência. A queda média do escore de disfagia entre os períodos pré e pós-operatório imediato foi de 7,33 pontos, o que representa diminuição de 81,17%. Conclusões: A redução de mais de 60% da pressão do esfíncter esofágico inferior entre os períodos pré e pós-operatório sugere que ela é preditora de bom prognóstico cirúrgico. O tratamento cirúrgico foi capaz de contribuir na qualidade de vida e alterar em muito a disfagia em longo prazo.

background: Laparoscopic nissen fundoplication is the surgical procedure of choice for patients with gastroesophageal reflux. Aim: To describe the most important surgical stages of the technique and report the rates of complications and mortality. material and methods: In a ten years period, 530 patients were subjec-ted to laparoscopic fundoplication. In all patients a clinical history was obtained and an upper endoscopy with biopsy, esophageal manometry and 24 h pH measurement were performed. Patients with Barret esophagus, hiatal hernia of more than 5 cm and those subjected to other surgical techniques were excluded from analysis. Results: No patient died. The conversion rate was 0.4%. No patient required splenectomy. Two patients had complications and required a second intervention. Mean hospital stay was 2.8 days. Conclusions: Laparoscopic nissen fundoplication is safe and has a low rate of complications.


Objetivo: El presente estudio pretende describir las etapas quirúrgicas más importantes de esta técnica y describir la morbimortalidad publicada por autores nacionales. material y método: Entre enero de 1993 y diciembre de 2013 un total de 530 pacientes se sometieron a una fundoplicatura laparoscópica dentro de un protocolo de estudio prospectivo. En todos se realizó una encuesta clínica, endoscopia con toma de biopsia, manometría esofágica y pH de 24 h. Se excluyeron pacientes con esófago de Barrett, pacientes con hernia hiatal > 5 cm y pacientes sometidos a una técnica quirúrgica diferente. Resultados: La mortalidad operatoria fue cero. La tasa de conversión fue de 0,4%. No hubo esplenectomía. Hubo un total de 2 pacientes complicados (0,4%), siendo todos reoperados. La estadía hospitalaria fue de 2,8 días. Conclusión: La fundoplicatura de Nissen por vía laparoscópica es un procedimiento seguro, de muy baja morbilidad y nula mortalidad operatoria.

Introducción: la hernia hiatal es una enfermedad adquirida y de las más frecuentes que afectan el aparato digestivo superior. El término hernia hiatal gigante se utiliza cuando el contenido de la hernia supera un tercio del estómago (30 por ciento en el tórax), o tienen un diámetro mayor de 5 cm. Objetivo: describir los resultados obtenidos en el tratamiento quirúrgico de la hernia hiatal gigante realizado por cirugía laparoscópica en el Centro Nacional de Cirugía de Mínimo Acceso. Métodos: se realizó un estudio retrospectivo, descriptivo y longitudinal, donde se incluyeron todos los pacientes intervenidos quirúrgicamente por hernia hiatal gigante en el Centro Nacional de Cirugía de Mínimo Acceso en el período comprendido entre enero de 2011 a diciembre de 2013. Resultados: la hernia tuvo un tamaño medio de 7 cm con rangos entre 6 y 15 cm. La técnica realizada con mayor frecuencia fue la herniorrafia hiatal más fundoplicatura de Nissen-Rossetti en 70 por ciento de los pacientes. Hubo 3 recidivas que requirieron reintervención y no hubo fallecidos. Conclusiones: la cirugía videolaparoscópica es la técnica de elección para el tratamiento de la hernia hiatal gigante, siempre que sea realizada por equipos quirúrgicos que tengan experiencia en procederes laparoscópicos de avanzada, lo que permite obtener resultados satisfactorios en cuanto a morbilidad y mortalidad(AU)


Introduction: hiatal hernia is an acquired disease and it is common in the upper gastrointestinal tract. The expression Giant hiatal hernia is called a hernia that exceeds one third of the stomach (30 por ciento in the thorax) and measures over 5cm diameter. Objective: to describe the results of the laparoscopic surgery for the treatment of giant hiatal hernias at the National Center of the Minimal Access Surgery. Methods: a retrospective, descriptive and longitudinal study was performed on patients with giant hiatal hernia and surgical treatment between January 2011 and December 2013 at the National Center for Minimal Access Surgery in Havana, Cuba. Results: the hernia size was 7 cm as average, ranging from 6 to 15 cm. The most used technique was hiatal herniorrhaphy plus Nissen-Rossetti funduplication in 70 por ciento of patients. There were 3 recurrences that required reoperation. No deaths were observed in this series. Conclusions: laparoscopic surgery is the best technique for the treatment of giant hiatal hernia, provided that the surgical teams have enough experience in advanced laparoscopic procedures, which can allow good results in terms of morbidity and mortality rates(AU)

BACKGROUND: Asthma, laryngitis and chronic cough are atypical symptoms of the gastroesophageal reflux disease. AIM: To analyze the efficacy of laparoscopic surgery in the remission of extra-esophageal symptoms in patients with gastroesophageal reflux, related to asthma. METHODS: Were reviewed the medical records of 400 patients with gastroesophageal reflux disease submitted to laparoscopic Nissen fundoplication from 1994 to 2006, and identified 30 patients with extra-esophageal symptoms related to asthma. The variables considered were: gender, age, gastroesophageal symptoms (heartburn, acid reflux and dysphagia), time of reflux disease, treatment with proton pump inhibitor, use of specific medications, treatment and evolution, number of attacks and degree of esophagitis. Data were subjected to statistical analysis, comparing the pre- and post-surgical findings. RESULTS: The comparative analysis before surgery (T1) and six months after surgery (T2) showed a significant reduction on heartburn and reflux symptoms. Apart from that, there was a significant difference between the patients with daily crises of asthma (T1 versus T2, 45.83% to 16.67%, p=0.0002) and continuous crises (T1, 41.67% versus T2, 8.33%, p=0.0002). CONCLUSION: Laparoscopic Nissen fundoplication was effective in improving symptoms that are typical of reflux disease and clinical manifestations of asthma. .


RACIONAL: A asma, a laringite e a tosse crônica são sintomas atípicos da doença do refluxo gastroesofágico. OBJETIVO: Analisar a eficácia da cirurgia laparoscópica na remissão de sintomas extra-esofágicos em doentes com refluxo gastroesofágico, relacionada com a asma. MÉTODOS: Foram revisados ​​os prontuários de 400 doentes com doença do refluxo gastroesofágico submetidos à fundoplicatura a Nissen entre 1994 e 2006 e foram identificados 30 casos com sintomas extra-esofágicos relacionadas à asma. As variáveis ​​consideradas foram: sexo, idade, sintomas gastroesofágicos (azia, refluxo ácido e disfagia), o tempo da doença do refluxo, o tratamento com inibidores da bomba de prótons, o uso de medicamentos específicos, tratamento e a evolução, o número de crises e o grau de esofagite. Além disso, houve diferença significativa entre os doentes com crises diárias de asma (T1 versus T2, 45,83% para 16,67%, p=0,0002) e crises contínuas (T1 41,67% versus T2 8,33%, p=0,0002). CONCLUSÃO: A fundoplicatura à Nissen por via laparoscópica foi eficaz na melhora dos sintomas que são típicos da doença do refluxo e manifestações clínicas da asma, melhorando a qualidade de vida. .

Context Evaluating quality of life after surgery for gastroesophageal reflux is fundamental, since improvement of symptoms is the main goal of treatment. Objective To compare the satisfaction of patients operated for GERD, interviewed at the office, with patients interviewed by telephone. Methods One hundred and seventy-eight patients were submitted to laparoscopic Nissen fundoplication. Ninety patients were found, and 45 agreed to come to the hospital’s office. The other 45 patients were interviewed by telephone. Quality of life was evaluated using three methods: The GERD-HRQL questionnaire, translated to Portuguese; question “If you could choose it today, would you do surgery again?”; and question “If you could classify your symptom improvement between 0 and 10, which grade would you give?” Patients were divided in group A (office interview) and B (telephone interview). Statistical analysis was performed. Results The average GERD-HRQL score in group A was 6.29, while in group B was 14.09(P = 0.002). There was no statistical difference between groups A and B regarding patients choice of doing the surgery again(P = 0.714) and their grade of postoperative symptom improvement (P = 0.642). Conclusion The overall postoperative satisfaction was high. The GERD-HRQL questionnaire might not be suitable for application by telephone in our population, and based on other analyzes, the satisfaction and quality of life of patients interviewed at the office or by telephone was similar. .


Contexto A avaliação da qualidade de vida após a cirurgia para a doença do refluxo gastroesofágico é fundamental, pois a melhora dos sintomas é o principal objetivo do tratamento. Objetivo Verificar a satisfação dos pacientes após a cirurgia para doença do refluxo gastroesofágico, e comparar a aplicação ambulatorial e telefônica do questionário GERD-HRQL. Métodos Cento e setenta e oito pacientes foram submetidos à fundoplicatura laparoscópica à Nissen. Noventa pacientes foram encontrados e 45 puderam comparecer ao ambulatório. Os demais 45 pacientes foram entrevistados por telefone. A qualidade de vida foi avaliada por três métodos: o questionário GERD-HRQL, traduzido para o Português; pergunta “Se você pudesse escolher hoje, faria a cirurgia novamente?”; e pergunta: “Se você pudesse classificar sua melhora dos sintomas entre 0 e 10, que nota você daria?” Os pacientes foram divididos em grupo A (entrevista ambulatorial) e B (entrevista telefônica). Resultados A pontuação média do grupo A no questionário GERD-HRQL foi de 6,29, enquanto que no grupo B foi de 14,09 (P = 0,002). Não houve diferença estatística entre os grupos A e B em relação a escolha de fazer a cirurgia novamente (P = 0,714) e o grau de melhora dos sintomas no pós-operatório (P = 0,642). Conclusão A satisfação geral dos pacientes no pós-operatório foi elevada. O questionário GERD-HRQL pode não ser adequado para aplicação por telefone na nossa população, e com base em outras análises, a satisfação e a qualidade de vida dos pacientes entrevistados no ambulatório ou por telefone foi semelhante. .

Context The occurrence of severe dysphagia after laparoscopic total fundoplication is currently an important factor associated with loss of quality of life in patients undergoing this modality of treatment for gastroesophageal reflux disease. Objectives Compare the incidence and evaluate the causes of severe postoperative dysphagia in patients undergoing laparoscopic total fundoplication (LTF) without short gastric vessels division, using the anterior gastric wall (Rossetti LTF) or anterior and posterior gastric walls (Nissen LTF). Methods Analysis of the data of 289 patients submitted to LTF without short gastric vessels division from January 2004 to January 2012, with a minimum follow-up of 6 months. Patients were divided in Group 1 (Rossetti LTF – n = 160) and Group 2 (Nissen LTF – n = 129). Results The overall incidence of severe postoperative dysphagia was 3.11% (4.37% in group 1 and 1.55% in group 2; P = 0.169). The need for surgical treatment of dysphagia was 2.5% in group 1 and 0.78% in group 2 (= 0.264). Distortions of the fundoplication were identified as possible causes of the dysphagia in all patients taken to redo fundoplication after Rossetti LTF. No wrap distortion was seen in redo fundoplication after Nissen LTF. Conclusions The overall incidence of severe postoperative dysphagia did not differ on the reported techniques. Only Rossetti LTF was associated with structural distortion of the fundoplication that could justify the dysphagia. .


Contexto A ocorrência de disfagia grave após fundoplicatura total laparoscópica representa atualmente um importante fator associado à perda da qualidade de vida em pacientes submetidos a esta modalidade de tratamento para doença do refluxo gastroesofágico. Objetivos Comparar a incidência e avaliar as possíveis causas da disfagia grave pós-operatória em pacientes submetidos à fundoplicatura total laparoscópica (FTL) sem liberação dos vasos gástricos curtos utilizando apenas a parede gástrica anterior (FTL à Rossetti) ou as paredes gástricas anterior e posterior (FTL à Nissen). Métodos Análise de dados de 289 pacientes submetidos à FTL sem liberação dos vasos gástricos curtos no período de janeiro de 2004 a janeiro de 2012, com acompanhamento mínimo de 6 meses. Os pacientes foram divididos em Grupo 1 (FTL à Rossetti – n = 160) e Grupo 2 (FTL à Nissen – n = 129). Resultados A incidência global de disfagia grave pós-operatória foi de 3,11%, sendo 4,37% no grupo 1 e 1,55% no grupo 2 (P = 0.169). A necessidade de tratamento cirúrgico da disfagia foi de 2,5% no grupo 1 e 0,78% no grupo 2 (P = 0.264). Em todos os pacientes reoperados após FTL à Rossetti foram observadas distorções das válvulas que podem responder pela ocorrência de disfagia, ao passo que no paciente submetido à reoperação após FTL à Nissen nenhuma alteração foi observada. Conclusões A incidência global de disfagia grave pós-operatória não apresentou diferença entre as duas técnicas relatadas. A FTL à Rossetti foi associada a distorções estruturais das válvulas que podem justificar a disfagia, fato não observado após FTL à Nissen. .

RACIONAL: A doença do refluxo gastroesofágico e sua recidiva podem estar relacionadas ao ganho de peso tardio após a cirurgia bariátrica, aumentando a incidência de metaplasia colunar e neoplasia. A plicatura gástrica associada à fundoplicatura pode ser empregada com o objetivo de se tratar a doença do refluxo e o sobrepeso. OBJETIVOS: Avaliar o hormônio grelina no pré e pós-operatório, bem como a perda de peso e o controle da doença do refluxo em pacientes submetidos à gastroplicatura com fundoplicatura. MÉTODOS: Foi realizada gastroplicatura com fundoplicatura videolaparoscópica em oito pacientes. Os exames endoscópicos foram realizados no pré e no pós-operatório, bem como a coleta de sangue para a dosagem do hormônio grelina. RESULTADOS: Houve melhora dos sintomas do refluxo e das alterações na mucosa em todos os pacientes. A perda de peso foi significativa. Não houve significância na redução dos níveis plasmáticos de grelina. CONCLUSÕES: Com a plicatura gástrica e fundoplicatura: 1) não houve redução significativa no hormônio grelina, apesar do relato de diminuição da fome e aumento da saciedade pelos pacientes; 2) o procedimento foi eficaz no tratamento da doença do refluxo; 3) obteve-se perda de peso significativa.


BACKGROUND: The recurrence of the gastroesophageal reflux disease may be related to later postoperative weight gain, therefore increasing the chances of developing columnar metaplasia and cancer. The gastric plication associated with fundoplication can be employed in order to be treating the two issues. AIM: To evaluate the serum ghrelin hormone in preand postoperatively as well as weight loss and control of reflux disease in patients undergoing gastroplication associated with fundoplication. METHODS: Was performed laparoscopic gastric plication with fundoplication in eight patients; endoscopic examinations were performed pre and postoperatively as well as blood collection for ghrelin hormonal dosage. RESULTS: There was control of reflux symptoms and mucosal lesions. Weight loss was significant. Since the change of the hormone ghrelin was not of great significance. CONCLUSIONS: Gastric plication associated with fundoplication was effective in treating reflux disease with surgical indication and for weight loss in obese patients. Appetite control occurs, but not due to ghrelin, because no significant decrease of its plasmatic levels was observed.

Background: Laparoscopic Nissen fundoplication is the most commonly used surgical technique for the treatment of gastroesophageal reflux. Method: This is a review of publications about long term results of the technique. Results: Thirteen papers reporting results at 10 years were identified. Heartburn recurrence was documented in 20 to 23 percent of patients. Symptoms such as bloating were reported by 47 percent of patients after the fundoplication. After 10 years, 25 percent of patients started to use proton pump inhibitors and a new surgical intervention was required by 9 percent. In eight reports, there was no objective assessment of surgical results. In one report only few cases had a radiological assessment. Only in three European reports and one report by us, endoscopy and esophageal manometry were performed. Conclusions: It is concluded that Nissen fun-doplication achieves a relief of gastroesophageal reflux at 10 years in 75 percent of patients. There is a paucity of reports carrying out objective assessments of surgical results.


Introducción: La fundoplicatura de Nissen por vía laparoscópica es la técnica más empleada en pacientes con reflujo gastroesofágico. Objetivo: Revisar las publicaciones referentes a los resultados alejados (10 o más años) de esta cirugía. Método: Se realizó una búsqueda bibliográfica en la literatura inglesa de todos los artículos referentes a este tema. Resultados: Se encontraron en total 13 trabajos relacionados con los resultados a más de 10 años. La recurrencia de la pirosis se presentó entre 20 a 23 por ciento, la aparición de síntomas post fundoplicatura, como el meteorismo, en un 47 por ciento. A los 10 años cerca de 25 por ciento comenzó a ingerir nuevamente inhibidores de la bomba de protones y hubo necesidad de una reoperación en un 9 por ciento. En 8 trabajos no hubo ninguna evaluación objetiva de los resultados quirúrgicos. En 1 trabajo sólo evaluaron radiología en algunos casos. Sólo en 3 trabajos europeos y 1 estudio nuestro se realizaron evaluaciones objetivas, con endoscopia y manometría esofágica. Conclusiones: La fundoplicatura de Nissen es una técnica quirúrgica que controla el reflujo gastroesofágico en el 75 por ciento de los pacientes a más de 10 años de la cirugía. Sin embargo, muy pocos trabajos efectúan evaluaciones objetivas, en la mayoría de las publicaciones, las evaluaciones son sólo sintomáticas y subjetivas.

CONTEXT: Gastroesophageal reflux diasease (GERD) is a highly prevalent disease. Treatment is divided into lifestyle modifications, medical and surgical treatment. Surgical laparoscopy is the gold standard treatment. In the last decade, there were an extensive research on procedures, less aggressive than laparoscopy and with better esthetic results. Minilaparoscopy is "reemerging" as a safe, effective and with excellent cosmetic results in selected patients treated for gastroesophageal reflux diasease. We present a serie of 27 patients treated for GERD by minilaparoscopic laparoscopy. MATERIAL: Between October 2009July 2011 a total of 27 patients underwent fundoplication by minilaparoscopy. It is used one 10mm trocar, a telescope of 30 degrees and four 3 mm trocars at regular positions. Regular surgical steps are done with no modifications. Cardiac tape, suture needles, and eventually extracting bag, gauze, are placed and taked out through the umbilical port. With these technical adjustments, we can perform the procedure safely and effectively, similarly to standard laparoscopic technique. RESULTS: Of the 27 patients, 22 were female and 5 male. The average body mass index was 25.5 kg/m². Hiatal hernias were small (<3 cm) in 24 patients. Mean operative time was 60 minutes. In all cases the hiatoplasty was performed with simple or 'x' stiches of 2.0 Ethibond. There was no need for conversion to standard laparoscopy or open surgery. The length of hospital stay was less than or equal to 24 hours in all patients. In this series of patients there were no postoperative complications. We did not observe any complication of the surgical wound. There were no evidence of recurrence of symptoms or endoscopic changes. CONCLUSION: Hiatoplasty associated with fundoplication using minilaparoscopic instruments is safe, feasible and effective. If compared to other "new access", has a spectacular esthetic results. Can be done with only minor technical adjustments, for any experienced laparoscopic surgeon, and is perfectly adaptable to our financial reality.


CONTEXTO: A doença do refluxo gastroesofágico (DRGE) é uma doença altamente prevalente. O seu tratamento é dividido em modificações de estilo de vida, tratamento médico e cirúrgico. A cirurgia laparoscópica é o tratamento padrãoouro. Nas últimas décadas houve uma extensa pesquisa sobre procedimentos menos agressivos do que a laparoscopia e com melhores resultados estéticos. A minilaparoscopia vem "reemergindo" como método seguro, eficaz e com excelentes resultados estéticos em pacientes selecionados, tratados para DRGE. É apresentada uma série de 27 pacientes tratados para a DRGE por minilaparoscopia. MÉTODOS: Entre outubro de 2009 e julho de 2011, o total de 27 pacientes foi submetido a fundoplicatura por videominilaparoscopia. Foram utilizados um trocarte de 10 mm, um telescópio de 30 graus e quatro trocarteres de 3 mm nas posições regulares. Os passos cirúrgicos são feitos sem modificações, de maneira habitual. Fita cardíaca, agulhas de sutura e, eventualmente, saco extrator e gaze são colocados e retirados através do portal umbilical. Com esses ajustes técnicos, podese realizar o procedimento de forma segura e eficaz, semelhantemente à técnica laparoscópica padrão. CONCLUSÃO: Hiatoplastia associada à fundoplicatura laparoscópica, utilizandose de instrumentos minilaparoscópicos é método seguro, viável e eficaz. Se comparado a outros "novos acessos", tem resultado estético espetacular. Pode ser realizado com apenas pequenos ajustes técnicos, por qualquer cirurgião experiente em laparoscopia e é perfeitamente adaptável a nossa realidade financeira.

PURPOSE: To analyze the behavior of cardiopulmonary function in postoperative of laparoscopic Nissen fundoplication. METHODS: Thirty-two patients, 13 males (41%) and 19 females (59%), were evaluated. Their age ranged from 25 to 67 years, with a mean of 44.4 ± 10.9. Pulmonary volumes, respiratory pressures and exercise tests were performed in the preoperative period (PRE) and in the first (PO1), second (PO2), fifth (PO5) and thirtieth (PO30) postoperative periods. RESULTS: Thirty-two patients were evaluated, of whom 59% were females. Mean age was 44.4 ± 10.9 years. Lung volumes had significant decrease at PO1 and PO2 and were similar to PRE values at PO5. Respiratory pressures were altered only at PO1. The distance covered in the 6-minute walk test had significant reduction until PO2, and climbing time in the stair-climbing test significantly increased at PO2. CONCLUSION: Patients submitted to LNF surgery have decreased cardiorespiratory function in the early postoperative period; however, they soon return to preoperative conditions.


OBJETIVO: Analisar o comportamento da função cardiopulmonar no pós-operatório de fundoplicatura Nissen laparoscópica. MÉTODOS: Trinta e dois pacientes, 13 homens (41%) e 19 mulheres (59%) foram avaliados. A idade variou de 25 to 67 anos, com media de 44.4 ± 10.9. Os volumes pulmonares, as pressões respiratórias e testes de exercício foram realizados no período pré-operatório (PRE) e no primeiro (PO1), segundo (PO2), quinto (PO5) e trigésimo (PO30) períodos pós-operatórios. RESULTADOS: Trinta e dois pacientes foram avaliados, dos quais 59% eram do sexo feminino. A média de idade foi de 44,4 ± 10,9 anos. Os volumes pulmonares apresentaram diminuição significativa no PO1 e PO2 e foram semelhantes aos valores PRE no PO5. As pressões respiratórias estiveram alteradas apenas no PO1. A distância percorrida no teste da caminhada de 6 minutos apresentou redução significativa até o PO2, e o tempo de subida no teste de escada aumentou significativamente no PO2. CONCLUSÃO: Pacientes submetidos à FLN apresentaram diminuição da função cardiorrespiratória no início do período pós-operatório, no entanto, elas rapidamente regressaram às condições pré-operatórias.

ABSTRACT

RACIONAL: A doença do refluxo gastroesofágico é uma das enfermidades do trato gastrointestinal mais frequentes atualmente. A fundoplicatura de Nissen-Rossetti é técnica operatória muito empregada para o tratamento cirúrgico, sendo a migração intratorácica da válvula uma das principais complicações. OBJETIVO: Avaliar a incidência da migração da fundoplicatura e seus fatores de risco. MÉTODO: Analisou-se retrospectivamente 207 prontuários de pacientes submetidos à fundoplicatura videolaparoscópica pela técnica de Nissen-Rossetti para tratamento da doença do refluxo. As variáveis analisadas foram: sexo, idade, esofagite, grau de esofagite, tamanho da hérnia, alargamento da cárdia, encurtamento esofágico. Para as variáveis quantitativas, foi considerado o teste t de Student. Para as nominais, foi considerado o teste de Qui-quadrado ou o teste exato de Fisher. Valores de p<0,05 indicaram significância estatística. RESULTADOS: Do total, 135 eram mulheres (65,22%) e 72 homens (34,78%), com média de idade de 47,43 anos. O tamanho da hérnia variou entre 2 e 6 cm. Duzentos pacientes tinham esofagite (96,62%), sendo 113 (56,50%) grau I, 75 grau II (37,50%) e 12 grau III ou IV (6%). Alargamento de cárdia e esôfago de Barrett foram vistos em 153 (73,91%) e 13 (6,28%) casos, respectivamente. Um paciente apresentava encurtamento esofágico. Dentre as mulheres, 33 (24,4%) apresentaram migração e entre os homens, apenas seis (8,3%) (p = 0,005). A idade média dos pacientes com e sem migração foi de 54,03 e 45,89 anos, respectivamente (p = 0,001). CONCLUSÃO: A incidência da migração da fundoplicatura foi de 18,8%. O gênero feminino e possuir maior idade influenciam na probabilidade dessa migração. O grau de esofagite, tamanho da hérnia e alargamento da cárdia não se mostraram fatores de risco para migração intratorácica da fundoplicatura.


BACKGROUND: Gastroesophageal reflux is the gastrointestinal tract disease most frequently find nowadays. The Nissen-Rossetti fundoplication is widely used for the surgical treatment, and intrathoracic migration of the valve is the most frequent complication. AIM: To assess the incidence of the fundoplication and its risk factors. METHODS: Were analyzed retrospectively medical records of 207 patients undergoing laparoscopic fundoplication by the Nissen-Rossetti technique for the treatment of reflux disease. The variables analyzed were: sex, age, esophagitis grade, size of the herniation, enlargement of the cardia and esophageal shortening. For quantitative variables, was considered the Student's t test. For the nominal, was considered the chi-square or Fisher's exact test. P values ​​<0.05 were considered statistically significant. RESULTS: Of the total, 135 were women (65.22%) and 72 men (34.78%) with mean age of 47.43 years. The size of the hernia varied between 2 and 6 cm. Two hundred patients had esophagitis (96.62%) and 113 (56.50%) grade I, 75 grade II (37.50%) and 12 grade III or IV (6%). Enlargement of the cardia and Barrett's esophagus were seen in 153 (73.91%) and 13 (6.28%) cases, respectively. One patient had esophageal shortening. Among women, 33 (24.4%) showed migration and among men, only six (8.3%) (p = 0.005). The average age of patients with and without migration was 54.03 and 45.89 years, respectively (p = 0.001). CONCLUSION: The incidence of the fundoplication migration was 18.8%. The gender (female) and higher age influence the probability of migration. The degree of esophagitis, size of hernia and enlargement of the cardia were not risk factors for intrathoracic migration of the fundoplication.
